- Location of incident (street address or intersection)
- Type of incident (medical aid, structure fire, vehicle accident, etc.)
The requested report will be sent to you via e-mail.
Subpoenas for records may be submitted via email or in person. For allied agencies seeking reports without a subpoena, please submit a signed request on your department's letterhead to email@example.com.
